Part Time up to 29 hours per week.

Must be flexible with schedule.

This individual is responsible for safely transporting patients to and from outside appointments as well as to and from treatment.

  • Evaluates own performance and sees opportunity for growth.

  • Participates in continuing education program conducted by facility.

  • Participates in annual needs assessment for department.

  • Adheres to facility confidentiality policies and procedures.

  • Displays a cooperative and positive attitude towards callers, patients, staff and visitors.

  • Maintains facility attendance policy of no more than 6 tardies or 3 incidents of unexcused/excused absences in a 90 day period (not FMLA).

  • Adheres to facility dress code.

  • Adheres to facility standards of conduct.

  • Adheres to facility safety policies and procedures.

  • Adheres to facility patient rights.

  • Adheres to facility infection control policies and procedures.

  • Adheres to facility drug free workplace.

  • Adheres to work and activity scheduled, i.e., group, seminar, staffing, department meeting and supervisor meeting.

  • Direct observation

  • Input systems for handling workload

  • Attendance at facility/departmental meetings

  • Exhibits clear concise verbal and written communication as observed by direct observation.

  • Demonstrates telephone skills, answering, taking messages, placing on hold and transferring calls.

  • Participates on facility’s Disaster Team.

  • Ensures patient safety.

  • Drives facility vehicle to transport patients to and from outside appointments.

  • Drives facility vehicle to transport patients to and from treatment.

  • Drives facility vehicle to pick up supplies, equipment as needed.

  • Insures all passengers in vehicle are wearing seatbelts.

  • Drives facility vehicles within state and federal driving laws, ie. Speed limits, school crossings, school buses, stop signs, passing, etc.

  • Insures facility vehicles are cleaned after each transport.

  • Strives to be available when called to provide transportation.

  • Provides transportation “on-call” (carries cell phone) as necessary.

  • Insures that patients coming into treatment are comfortable and safe.

  • Exhibits ability to report pertinent patient information to appropriate call center, clinical and/or nursing staff.

  • Insures transportation efforts are carried out in a safe, but timely manner.

  • Maintains professional relationship with all patients.

  • Maintains transportation log.

  • Coordinates with maintenance department to insure proper maintenance of vehicles is maintained.

  • Insures Fire Extinguisher, First Aid Kit, and Accident Kit are properly maintained and secured in vehicle.

  • Other duties as assigned.


Screenings - Willing to submit to pre-employment procedures including drug screen and background check.

Education – High School Diploma or GED required.

Experience – Knowledge/experience working with patients who are chemically dependent helpful, but not required.

Certifications - CPR/CPI (offered on site).

Licenses – Valid Florida Driver’s License and good driving record.

Special Requirements – Knowledge of 12 Step Recovery Programs.

Age/Gender Specifics - Ability to work with male and female, adult (18-over) substance abuse/dual diagnosed population.

Physical/Emotional Demands/Working Conditions:

Work is primarily sedentary with long periods of sitting in vehicle while transporting. Requires some lifting of 30 or more pounds (luggage, packages, etc.). Must be able to support patient weight in case of emergency situation or patient disability requiring assistance in walking, etc. Must be emotionally stable and exhibit ability to display coping skills. Exposed to risk of highway accidents. Risk of exposure to infections and contagious/blood borne diseases. Exposed to varying and unpredictable situations (i.e., patient behavior).
